Dear Principal,
The Office of Fitness and Health Education is excited to inform you that the application period for the Winter C.H.A.M.P.S. SEASON is now open!
C.H.A.M.P.S. (Cooperative, Healthy, Active, Motivated, Positive, Students) provides opportunities beyond the school day for middle-level students to become interested in their own wellbeing and participate in a variety of organized before and/or after-school fitness and sports activities.
The following step-by-step guide will walk you through our web-based application process.
*NOTE: Principals are the only persons with access to this application.
STEP 1: LOG-IN
- Log onto the following link using your DOE outlook email address and password:

- Paper applications and/or email applications will not be accepted. The application will remain open until 5:00 PM Monday, December 15, 2008. Applications will not be accepted after the deadline.
STEP 2: CHOOSE ACTIVITITES
- Choose the activities that you wish to have for the Winter season
- You may select up to 2 activities from the following list:

STEP 3: RECOMMEND TEACHER-COACH
- Select the teacher that you are recommending as the coach for each activity – there is a “drop down” list of all of the teachers in your building that you may select from.
- If there is a teacher from another school that you would like to recommend, you will be prompted to email their information to our office.
- Enter the expiration date of the teacher’s AED/CPR certification.
- If the teacher doesn’t have current AED/CPR certification, they will need to obtain this prior to the start each Season. Visit www.CHAMPSnyc.org for a list of available workshops.
- Each teacher may only be recommended for one activity per season.
- Once this information is entered click “SAVE.”
- You will receive an email receipt stating that your application has been received. The teacher(s) you recommended will receive a similar email to their DOE outlook account.
STEP 4: ADVERTISE THE COACHING POSITION
- Post a per-session position in your school to advertise the coaching position(s) by December 2, 2008. A sample template with eligibility and minimum criteria requirements is attached to this letter, and is also available on the home page of www.CHAMPSnyc.org under “What’s New.”
- The teachers will be applying directly to you. They will need to submit:
o Signed OP 175
o Copy of current AED/CPR certification
o Copy of NYC DOE Teaching license
o Resume and cover letter
o These documents will need to be kept on file at the school
STEP 5: NOTIFICATION
- Completion of the application does not guarantee a program. The Office of Fitness & Health Education will contact you to let you know if you’ve been awarded program(s) on or before December 23, 2008.
- Priority will be given to schools with NYC FITNESSGRAM completion rate of 75% or higher.
- Once your application is approved, an Award Letter will be emailed to you confirming the programs. School payroll secretaries will also be contacted regarding how he/she will be able to access the per-session funds and understand the C.H.A.M.P.S. payroll process.
