NYS LOAN REQUEST AND ENROLLMENT CERTIFICATES (LREC) & PRINCIPAL’S STATEMENT OF ASSURANCES REQUIREMENTS (PSAR)

The current NYS Loan Enrollment Certificates and the Principal's Statement of Assurances Guidelines and forms for School Year 2009-2010 are now available under NPS Memoranda.

NYS Software, Library and Instructional Computer Hardware Enrollment and Principal’s Statement of Assurances Requirements

This year in an effort to open the NYSSL (software) and NYSLIB (library) programs earlier in the fall, the NYSSL/NYSLIB & NYS CH Loan Request and Enrollment Certificate (Attachment B) and the (2) two Principal’s Statement of Assurances Requirements forms (Attachments C1 & C2) must both be returned to the office indicated no later than Friday, October 16, 2009. The NYSSL/NYSLIB & NYS CH enrollment data is based on your school’s previous October 31, 2008 register as reported to the State Education Department on your 2008-2009 BEDS report. Funding for these programs can not be allocated without receipt of all information requested and forms properly completed and duly signed.

Textbook (NYSTL) Enrollment

Your preliminary NYSTL allocation for early ordering, which commenced in June 2009, was based on your school’s BEDS data as submitted to the State Education Department (SED) last fall. The NYSTL Loan Request and Enrollment Certificate (Attachment A), which requests your schools enrollment data based on the current October 31, 2009 register must be returned no later than Friday, November 13, 2009. Upon receipt of your textbook LREC, adjustments will be made accordingly to your school’s NYSTL account if so warranted with appropriate documentation.

FY’10 NYSTL

The FY’10 NYSTL Loan Program for the submission of textbook orders for school year 2009-2010 is open. Please check your school’s spending plan to determine your NYSTL funding, then you may proceed to order in FAMIS if you have not already participated in early ordering. The NYSTL allocation remains at $58.25 per eligible student.

All orders placed for FY’10 for textbooks must be placed through the FAMIS portal using the E-Catalogs and/or the contract data sheets for Used Textbooks and/or Sheet Music under Quick Code: 012129. Please refer to the detailed ordering information provided in NYSTL Guidelines and Ordering Information, under NPS Memoranda.
