Requesting an Unpaid Non-Medical Leave

To request a leave of absence, you must complete the appropriate leave application. Obtain your principal or supervisor’s signature on the leave application, gather all supporting documentation and submit the completed packet to HR Connect’s Medical, Leaves & Records Administration. Forms and supporting documentation required depend on the type of leave requested and the type of employee requesting the leave.

Note that to apply for a non-medical leave of absence without pay (other than a military), instructional/school-based employees must use the Self-Service Online Leave Application System (SOLAS).

Note: Certain non-medical leaves, including childcare and care of a sick family member are FMLA-qualifying events. Eligible employees must apply for a FMLA leave of absence. For more information, click here.

    • Instructional / school-based Staff

      Instructional and school-based employees are entitled to apply for and be granted a non-medical leave of absence without pay. Requests for non-medical leaves of absence must be approved by your principal or supervisor and should be accompanied by required supporting documentation. Non-medical leaves of absence are granted for up to one year only. Employees must reapply each year.

      Important Tip: You should submit your leave application prior to the first day of absence.

      To apply for a non-medical leave of absence without pay (other than a military), Instructional/School-based employees must use the Self-Service Online Leave Application System (SOLAS).

      Log in using your DOE network/email ID username and password. (This is the same user name you use to log into the DOE network, and matches your email address.)

      You will be able to upload supporting documents directly into the system.

      If you are unable to upload your supporting documents, the system will generate a customized fax coversheet. Use this coversheet to submit the supporting documents via the dedicated SOLAS fax number: (718) 935-5175.

      **Employees applying for military leaves must continue to use the paper process and complete the OP 160 Leave of Absence without Pay Application form.

      Below is a chart of required supporting documentation and eligibility requirements based on leave type:

       Study  Bursar's receipt indicating at least 8 credit hours per semester at an accredited college or university  Appointed teacher-line staff member with 1 year of service  Up to 2 years, must reapply each year
      To work for a charter school A letter from the principal of the charter school confirming your offer of employment with the school, the position title, and the dates of the employment.

      IMPORTANT: The charter school must be affiliated with the NYC Department of Education.


      If the employee wants to work at a charter school not affiliated with NYC Department of Education, they must resign from their position.
      Appointed teacher-line staff member with 1 year of service Non-conversion charter schools: Up to 2 years with option to extend up to 1 additional year (total of 3 years); must reapply each year

      If employee is currently working at a school that is converting or if employee was working at the school during the conversion from NYC public school to NY public charter school: Up to 2 years with option to indefinitely extend; must reapply each year
      Adjustment of Personal Affairs  Letter detailing situation of grave nature  Appointed teacher-line staff member with 1 year of service One year with a one year extension
      Must reapply each year
      Military Service 

      Copy of military orders, Military Pay Reimbursement Agreement 

      Important: To apply for a leave for military service you must complete and sign the OP 160 Leave of Absence without Pay Application form. Obtain your principal's signature and submit the OP 160 along with the required supporting documentation by fax to HR Connect at 718-935-4350.

      No seniority requirement 
      Spouse in the Military  Copy of military orders, Military Pay Reimbursement Agreement 

      Important: To apply for a leave for military service you must complete and sign the OP 160 Leave of Absence without Pay Application form. Obtain your principal's signature and submit the OP 160 along with the required supporting documentation by fax to HR Connect at 718-935-4350.

      No seniority requirement 

      The Self-Service Online Leave Application System offers you the ability to upload your supporting documents and submit them with your online application. If you are unable to upload your supporting documents, the system will generate a customized fax coversheet. Use this coversheet to submit the supporting documents via the dedicated SOLAS fax number: (718) 935-5175.

      Once the leave application is processed, you will receive a determination from HR Connect Medical, Leaves and Records Administration.

    • Educational Leadership Staff

      To apply for a non-medical leave of absence without pay, Educational Leadership Staff employees must complete the OP 160  Leave of Absence without Pay Application form.

      When applying please ensure the following:

      • Complete the application form in its entirety
      • Sign and date the application form
      • Ensure that your principal or supervisor signs the application form
      • Include the required supporting documentation (see table below) 

      Important Tip: You should submit your leave of absence without pay application prior to the first day of absence, or as soon as possible thereafter. The most common delays in processing a leave of absence without pay are caused by incomplete and late applications.

      To apply for a non-medical leave of absence without pay, UFT Educational Paraprofessionals employees must complete the Educational Paraprofessional Leave Application Leave of Absence without Pay Application form.

      When applying please ensure the following:

      • Complete the application form in its entirety
      • Sign and date the application form
      • Ensure that your principal or supervisor signs the application form
      • Include the required supporting documentation (see table below)

      Family Paraprofessionals and Hourly Non-Competitive Employees who want to apply for a non-medical leave of absence must use the Family Paraprofessionals & Hourly Non-Competitive Employees Leave Application.

      When applying please ensure the following:

      • Complete the application form in its entirety
      • Sign and date the application form
      • Ensure that your principal or supervisor signs the application form
      • Include the required supporting documentation (see table below)

      Permanent administrative employees should apply for a non-medical leave using the form Application for a Non-Medical Leave of Absence - Administrative Employees.

      • Include the required supporting documentation (see table below)
