    The PT Evaluation Appendices (worksheets) have been updated. 
    They are similar to the old appendices but with designated spaces 
    to record adaptations and assistance, and with some improvements. 
    There are 2 versions - long and short worksheets. You may choose 
    to utilize whichever version you prefer. Both versions have the same 
    content, but the long version provides more space to write on. 
    The long version also contains more detailed instructions; therefore, 
    it is recommended to use this the first time you utilize the updated 
    appendices. *UPDATED 10/2011*
