Forms

All Forms are in Adobe Acrobat format unless otherwise stated. To view forms you will need to download the Adobe Acrobat Reader which is free of cost at Adobe Acrobat Reader. The prequalification application is in FormDOCS format. The form is packaged with the FormDOCS filler program required to view and fill in the form electronically.

A hotline, staffed by qualification specialists, is available to answer any questions regarding the Prequalification Application. The hotline telephone number is (718) 472-8777. In addition, you may visit the Contractor Qualification Unit, Monday through Friday, between 9:00 a.m. to 4:00 p.m. for individualized assistance with the Prequalification Application.

After you have printed the Prequalification Application and any applicable qualification statement(s), please fill them out and submit one (1) original and one (1) copy together with any attachments to:

SCA Safety Manual

The NYCSCA has established this “SCA Safety Manual” in order to guide and direct the management, staff, and contractors working on all NYC school buildings of their safety obligations. The NYCSCA is committed to safety and considers effective safety management a shared responsibility. The SCA Safety Manual’s objectives are to develop and maintain a safe and healthful workplace.
