The federal No Child Left Behind Act of 2001 allows parents/guardians of children who attend Title I funded schools to request information about the professional qualifications of their child’s classroom teachers and/or paraprofessional staff. Parents/guardians of children in Title I schools should have received a letter from the school principal advising them of their right to request this information.
To assist parents/guardians with children in Title I schools who elect to request this information, we have developed a request form that can be downloaded and printed in the parent’s native language by clicking on the appropriate link below.
To use this Parent Request Form, please follow these instructions:
· Download and print the form by clicking on the appropriate language
· Complete the portion of the form entitled “To Be Completed By Parent/Guardian”
· Forward the completed form to your child’s school principal
The form will be returned to you by mail after it has been completed by your child’s principal and regional personnel office.

Parent Right to Know Letter- Principals can use the following letters to inform parents/guardians of their right to request information on the professional qualifications of their child’s teacher and/or paraprofessional.
Parent 4 Week Letter- Principals can use the following letters to inform parents/ guardians that their child has been taught for four or more consecutive weeks by a teacher in a core academic subject who is not Highly Qualified. Letters must be sent individually to parents/ guardians to protect privacy.
