NYC 2012 Parent Surveys need to be completed by April 5th.  

You need your survey access code to complete.  This can found two ways:

1) Bottom of your survey form

OR

2) Call 1-800-690-8603 to request your code.  You will only need your student's first and last name to obtain the code

Please complete by April 5th.
