Dear Parents,

We are pleased to offer your child a place in the After School Math Test Prep Program. The After School Math Test Prep Program is free and open to all students. Students will have the opportunity to participate in a 1 hour, two times per week (Tuesday and Wednesday) from 3:00-4:00 PM, Math Test Prep class for the New York State Mathematics Test which will be administered the week of March 10, 2008. The test preparation classes will begin on Tuesday, January 29, and will continue through Wednesday, March 05, 2008. Students not in extended-day activities may utilize the Library, Homework Help in Room 119 or the Mac Computer Lab in Room 302 from 2:20-3:00 PM as they wait for the test prep program to begin at 3:00 PM.

Please FILL OUT and return this ENTIRE form to Ms. Saltsberg in room 219B during AM Homeroom on or before Wednesday, January 23, 2008. Students will be placed on a first come basis. Announcements will be made in school reminding children of the start of the program. Children will receive the bottom portion of this form with their room assignment.
