KINDERGARTEN PRE-REGISTRATION FOR
SCHOOL YEAR 2012-13
Dear Parents of Kindergarten 2012-13 children:
In order to be registered the child must be born in 2007.
Please be informed that registration for our Kindergarten class of September 2012 will begin on January 9, 2012. The parents of children born in the year 2007 may pick-up the application packet Monday-Friday between 9:00 - 11:00. When you pick up the application, please provide the following documentation:
Address documentation (2-3 pieces):
- Utility bill (gas, electric, water)
- Deed to a house of Mortgage Statement
- Letter from government agency like IRS, City Housing Authority, Human Recourses
- Pay stub, letter from social agency, community based organization, religious institution
- Bank Statement
Proof of Birth:
- Birth Certificate
- Baptismal Certificate
- Passport
Other Documents:
- Immunization records (4 DTAP, 3 OPV, 2 MMR, 3 HepB, 1 Varicella needed)
